Clean-out is designed to keep a temporary airtight seal on the plumbing system allowing access inside the sanitary drainage system for inspection and cleaning. Ideally, you would want this on the highest inlet exterior plumbing fixture location with a pipe extending beyond the building and trimmed down to fit flush would the exterior of the structure with a threaded cap in most instances. if there is a blockage you can remove the cap and use a sewer machine to clean and break up any debris that may have been collected and flush down the pipe. On a sewer line clean-out, you would install it on the building drain which starts at 18in beyond the structure on the sewer line leading to City sewage or a septic tank. Clean out on the outlet side of your home connecting to the sewer line ensures that you have access to the plumbing sewer line system for maintenance and inspection… To install a clean-out on a building connected to the sewer line you would first need to expose the sewer line remove a section of pipe and install a sweeping fitting to allow for directional scoping with a hand cable sewer machine or line inspection camera. this is called a combination fitting it is designed for two inlets and one outlet with the directional flow with no obstructions. .It is code to install a clean-out every 50 feet of sewer pipe. This also allows for locating relatively easily in most situations.
